About the job:

We are currently looking for experienced and Qualified Social Workers for an exciting 6 month project for Cambridgeshire County Council’s Adult Learning Disability Partnership.

The Project is to ensure that residents of Cambridgeshire (predominantly people with learning disabilities) continue access to a CHC service which offers an appropriate level of care.

The role is due to start in January 2020 and is flexible in its location across Cambridgeshire.

  • To review and assess care packages that fall within the scope of Continuing Health Care guidance,
  • To review and assess a small number of cases in out of area placements,
  • Refresh processes and procedures agree with partners and implement
  • Ensure the care packages have been correctly assessed and finance assigned as per care package,
  • Explore all packages that trigger the NHS – CHC National Guidance,
  • Present evidence to Cambridge & Peterborough Clinical Commissioning Board.
  • Thorough case management skills,
  • In depth knowledge of the CHC process,
  • Ability to work in a client/ person centred approach,
  • Ability to be organised and meet deadlines,
  • Availability for 6 months.
  • A minimum of 2 years post-qualifying experience in Adult’s services,
  • Experience of working with Adults and Continuing Healthcare,
  • Car driver,
  • Social worker qualification at degree level or equivalent,
  • HCPC registration,
  • Current enhanced DBS certificate valid to work with adults, or willingness to complete.
  • £400 ‘Golden Hello’* payment for new social workers
  • Holiday entitlement of up to 36 days per annum, pro rata (including bank holidays)
  •  Pension contribution from employer
  • ‘Refer a Friend’ bonus* – receive £250 for every qualified social worker that you refer who we successfully place
  • Mileage – 45p
